Dear Parent/ Guardian:
As a culminating activity to the bowling unit in our Physical
Education classes, we will be traveling to Sartell Great River Bowling Lanes to
bowl from 10:15 - 12:00 on Friday, December 21st. The cost of this event is $6.00 and parents are welcome to
stop by and watch us hone our skills. Please complete this permission slip and
return it to your child’s teacher on or before December 19th, so we can have an
accurate count of our attendance. Concessions and video games will not be
available during this time frame.
Since behavior is a valuable part of a school’s environment and a
child’s learning, we will expect that all students will demonstrate appropriate
classroom behavior between now (December 10th) and the holiday
break. Any students not following
our P.A.T.H. guidelines will not be able to attend the bowling field trip.
In addition, students with missing classroom work will not be
attending the December 21st event. We will hold a 45-minute make-up
session on December 20th, so students have an opportunity to
complete missing assignments. Please encourage your child to complete all tasks
in a timely manner. If you would like to help them complete these assignments,
please feel free to do so.

I grant, ____________________________________, permission to
attend the Sartell Great River Bowling event on December 21st and have included
$6.00 along with this permission slip for my child to attend. I understand that
he/she may not be able to attend if his/her behavior is deemed inappropriate prior
to or during the event. Also, I understand that he/she may not be able to
attend if he/she is missing any classroom work prior to the event.
